A certificate holder or licensee wishing to reinstate their certificate or license when the certificate or license has lapsed for non-renewal for more than 5 years and has not been actively practicing in another state shall:

1. (a) Meet all the requirements set forth in Alc 414.01; and
2. (b) Provide documentation directly from the IC&RC that the applicant has taken and passed the appropriate IC&RC examination.
