- (a) For each pre-approved continuing education program attended, licensees and certification holders shall retain, for no less than 6 years, a certificate of completion containing the information described in (b) below.
(b) Certificates of completion shall contain the following information:
- (1) Name of the attendee;
- (2) Title of the training;
- (3) Name of the presenter including their credentials;
- (4) Name of training provider;
- (5) Date of the training;
- (6) Number of hours the training provided;
- (7) Identification of whether the education was pre-approved; and
- (8) A description of the performance domains covered by the training, when applicable.
- (c) For each continuing education program attended that has not been pre-approved, licensees and certification holders shall retain, for no less than 6 years, supporting documentation as described in (d) below that demonstrates the program complies with Alc 409.01 through Alc 409.03.
- (d) Documentation sufficient to demonstrate an unapproved course’s compliance with the continuing education requirements of this section shall include a course syllabus or other similar documentation which contains the information described in (b)(1) – (6) above.
