1. (a) Licensees shall document each instance of peer collaboration with a photocopy of a written statement:

   1. (1) Including:

      1. a. The date of the peer collaboration;
      2. b. The printed names and licenses held by each participant; and
      3. c. The duration of the peer collaboration stated in hours and minutes; and
   2. (2) Signed by the other participant(s) in the peer collaboration.

2. (b) Licensees shall retain such documents until they are required to submit them to the OPLC:

   1. (1) As part of their applications for renewal of licensure; or
   2. (2) At any other time on the request of the board through the OPLC.
