The administrative obligation of the supervised CRSW during supervision shall be to:

1. (a) Maintain records of the dates and hours they receive supervision and the name(s) of the supervisor(s);

2. (b) On the basis of such records, prepare a cumulative statement containing the following:

   1. (1) For each week identified by calendar date, the number of hours of supervision received in each supervision session;
   2. (2) The credentials, printed name(s), signature(s) of the supervisor(s) providing the supervision, and the date the statement was signed by the supervisor; and
   3. (3) The printed name and signature(s) of the CRSW preparing the statement and the date of the signature; and
3. (c) Maintain records of supervision for a period no less than 7 years after the termination of the supervision.
