Licensed clinical supervisors who wish to renew their license shall:
- (a) Comply with the continuing education requirements described in Alc 409.03; and
- (b) Complete and submit the “Universal Application for License Renewal” required by Plc 304.01(a)(1) and the “Licensed Clinical Supervisor Addendum to the Universal Application for Licensure Renewal”.
