The applicant’s signature on the application form shall mean that:

1. (a) The applicant confirms that the information provided on the application form and submitted by the applicant to support his or her application is true, accurate and complete to the best of his or her knowledge and belief; and
2. (b) The applicant acknowledges that the knowing making of a false statement on the application form is punishable as a misdemeanor under RSA 641:3.
