The applicant’s signature on the “Initial Certification Application as a Recovery Support Worker” form shall mean that:

1. (a) The applicant confirms that the information provided on the “Initial Certification Application as a Recovery Support Worker” form and submitted by the applicant to support his or her application is true, accurate, and complete to the best of his or her knowledge and belief; and
2. (b) The applicant acknowledges that knowingly making a false statement on the “Initial Certification Application as a Recovery Support Worker” form shall be punishable as a misdemeanor under RSA 641:3.
