At least 5 days before the hearing, the parties and intervenors shall provide to the other parties and intervenors:

1. (a) A list of witnesses intended to be called at the hearing;
2. (b) A list of documents and exhibits intended to be offered as evidence at the hearing; and
3. (c) A copy of each document intended to be offered as evidence at the hearing.
