1. (a) The “Native Lumber Certification Form,” effective 3-31-26, when required, shall be completed by a representative of the sawmill and signed by a grader permitted in accordance with this section for every order of native lumber sold.

2. (b) The “Native Lumber Certification” form shall include:

   1. (1) The species of lumber to be sold;
   2. (2) The quantity sold in board feet;
   3. (3) The full address of the location of use;
   4. (4) An indication of whether the lumber is green or dry;
   5. (5) The name and location of the sawmill;
   6. (6) The name of the permitted grader; and
   7. (7) The date of sale.
