The information submitted about farm land and real estate offered for sale shall be on a completed department-issued form:

1. (a) Bearing the notarized signature of the property owner; and

2. (b) Including the following:

   1. (1) A description of the property to be advertised; and

   2. (2) A statement of certification that:

      1. a. The signer is not a real estate agent or real estate broker; and
      2. b. The signer is the present legal owner of the property described.
