1. (a) In addition to other requirements set by rule, statute, or other law, each registered service technician shall complete and submit “Placed in Service/Inspection Report Form TAXI 08-2007 (REV. 07/14)” to the department.

2. (b) A registered service technician shall furnish the following on “Placed in Service/Inspection Report Form TAXI 08-2007 (REV. 07/14)”:

   1. (1) Name of business;
   2. (2) Full address, including city, state, and zip code;

   3. (3) The following information about each taxi meter:

      1. a. Whether installed;
      2. b. Whether certified and the stamp number;
      3. c. Whether tested and calibrated for device license;
      4. d. Whether rejected;
      5. e. Whether repaired;
      6. f. Equipment number;
      7. g. Vehicle identification number;
      8. h. Registered plate number;
      9. i. Tire size;
      10. j. Make of meter;
      11. k. Serial number of meter;
      12. l. Model number of meter;
      13. m. Drop money;
      14. n. Plus money;
      15. o. Odometer reading-ending;
      16. p. Odometer reading-beginning;
      17. q. Total miles; and
      18. r. Total money value;

   4. (4) The following information about each test data:

      1. a. Type of test;
      2. b. Actual discharge rate;
      3. c. Totalizer reading start; and
      4. d. Totalizer reading finish;

   5. (5) The following information about each prover data:

      1. a. Prover temperature;
      2. b. Prover reading;
      3. c. Correction for expansion/contraction of prover;
      4. d. Volume correction for prover reading; and
      5. e. Prover reading corrected to 60 degrees Fahrenheit;

   6. (6) The following information about each meter data:

      1. a. Gross meter reading; and
      2. b. Net meter reading;

   7. (7) If an uncompensated run or systems that give both gross and net readings, the following:

      1. a. Average meter temperature;
      2. b. Volume correction for meter reading; and
      3. c. Meter reading corrected to 60 degrees Fahrenheit;
   8. (8) Net delivery error; and
   9. (9) Correct gross delivery error.
