1. (a) In addition to other requirements set by rule, statute, or other law, each registered service technician shall complete and submit form “Placed in Service/Inspection Report Form Vehicle Sales SCLGCAP REV. 8/10” to the department.

2. (b) A registered service technician shall furnish the following on form “Placed in Service/Inspection Report Form Vehicle Sales SCLGCAP REV. 01/07”:

   1. (1) Name of business;
   2. (2) Full address, including city, state, and zip code;

   3. (3) The following information about each vehicle tank meter at the business:

      1. a. Whether installed;
      2. b. Whether certified and stamp number;
      3. c. Whether repaired;
      4. d. Whether tested and calibrated;
      5. e. Whether rejected;
      6. f. Make of scale;
      7. g. Scale capacity;
      8. h. Serial number of scale;
      9. i. Make of beam or dial;
      10. j. Minimum grad;
      11. k. Make of indicator;
      12. l. Platform size;
      13. m. Type of levers; and
      14. n. Balance condition on arrival;
   4. (4) Section test as found;
   5. (5) Section test as sealed;

   6. (6) The following information about the load test results:

      1. a. Sensitivity requirement (“SR”) at zero;
      2. b. SR at maximum test load; and
      3. c. Scale indication;

   7. (7) The following information about each performance data as found and as sealed:

      1. a. Test load;
      2. b. Scale indication; and
      3. c. Error; and

   8. (8) The following information about the strain-load test from left to right and right to left:

      1. a. Indicate weight of empty truck;
      2. b. Amount of test weights added;
      3. c. Scale indication; and
      4. d. Error on test weighs only.
