1. (a) In addition to other requirements set by rule, statute, or other law, each registered service technician shall complete and submit “Placed in Service/Inspection Report Form VEHTANK 04-2005 (REV. 07/14)” to the department.

2. (b) A registered service technician shall furnish the following on form “VEHTANK 04-2005 (REV.07/14)”:

   1. (1) Name of business;
   2. (2) Full address, including city, state, and zip code;

   3. (3) The following information about each vehicle tank meter at the business:

      1. a. Whether installed;
      2. b. Whether certified;
      3. c. Whether it replaces a prior device, and the device’s serial number (SN#);
      4. d. Whether repaired;
      5. e. Whether tested and calibrated;
      6. f. Whether rejected;
      7. g. Equipment number;
      8. h. Make of meter;
      9. i. Serial number of meter;
      10. j. Size;
      11. k. Product used to test meter;
      12. l. Make of register; and
      13. m. Serial number of register;

   4. (4) If the following installation complies with NIST Handbook 44:

      1. a. Air eliminator and hose;
      2. b. Zero reset;
      3. c. Adjustments found sealed;
      4. d. Antidrain valve;
      5. e. Printer; and
      6. f. Data plate visible; and

   5. (5) The following information about each performance data as found and as sealed:

      1. a. Type of test;
      2. b. Gallons per minute;
      3. c. Meter indication;
      4. d. Prover reading; and
      5. e. Meter error.
