1. (a) In addition to other requirements set by rule, statute, or other law, each registered service technician shall complete and submit “Placed in Service/Inspection Report Form sclm/12-03 (Rev. 04/15)” to the department.

2. (b) A registered service technician shall furnish the following on “Placed in Service/Inspection Report Form sclm/12-03 (Rev. 04/15)”:

   1. (1) Name of business; and

   2. (2) Full address, including city, state, and zip code;

      1. a. The following information about each scale and/or linear measure at the business:
      2. b. Whether installed;
      3. c. Whether it replaces a prior device, and the device’s serial number (SN#);
      4. d. Whether repaired;
      5. e. Whether tested and calibrated;
      6. f. Whether rejected;
      7. g. Whether placed in service;
      8. h. Make of scale or linear measure;
      9. i. Model number of scale or linear measure;
      10. j. Serial number;
      11. k. Capacity; and
      12. l. Location or lane number.
