1. (a) In addition to other requirements set by rule, statute, or other law each registered service technician shall complete and submit “Placed in Service/Inspection Report Form LPG” Rev. 07/14 to the department.

2. (b) A registered service technician shall furnish the following on “Placed in Service/Inspection Report Form LPG” Rev. 07/14:

   1. (1) Name of business;
   2. (2) Full address, including city, state, and zip code;

   3. (3) The following information about the liquid petroleum gas meter:

      1. a. Make of meter;
      2. b. Serial number;
      3. c. Size;
      4. d. Minimum and maximum rated G.P.M.;
      5. e. Make of register;
      6. f. Register model number;
      7. g. Register serial number;
      8. h. Whether there is a temperature compensator;
      9. i. Thermometer well; and
      10. j. Vapor eliminator;

   4. (4) The following information about the test data:

      1. a. Type of test;
      2. b. Flow rate;
      3. c. Meter temperature 35 gallon;
      4. d. Meter temperature 70 gallons;
      5. e. Tank pressure;
      6. f. Prover pressure;
      7. g. Prover temperature;
      8. h. Prover reading to nearest 0.05 gallon and
      9. i. Meter reading;

   5. (5) If an uncompensated run, the following:

      1. a. Average meter temperature;
      2. b. Temperature difference;
      3. c. Temperature correction factor;
      4. d. Correction for temperature difference; and
      5. e. Temperature correction prover reading; and

   6. (6) If either an uncompensated or compensated run, the following:

      1. a. Correction for prover pressure reading;
      2. b. Correction for prover temperature;
      3. c. Corrected prover reading;
      4. d. Meter reading; and
      5. e. Net meter error, in cubic inches if desired.
