1. (a) A hearing shall be commenced by an order of the committee giving the parties the notice specified in paragraph (b).

2. (b) The hearing notice shall contain:

   1. (1) A statement of the time, place and nature of the hearing;
   2. (2) A statement of the legal authority under which the hearing is to be held;
   3. (3) A reference to the applicable statutes and rules;
   4. (4) A short and plain statement of the issues presented;
   5. (5) A statement that each party has the right to have representation by an attorney at the party’s own expense; and
   6. (6) The name of the presiding officer.
