1. I. Applications for licenses shall be made on forms adopted by the board.
2. II. Applications for licensure shall be accompanied by proof of the surety bond required by this chapter, and payment of the license fee.
3. III. The board may require each applicant to provide additional information about the applicant's background and trustworthiness as is reasonably necessary.
