4.5-02.1-04-02. Application for private practice registration.

1. Qualifications. To be eligible for registration to provide private practice, an individual must be licensed as a licensed clinical addiction counselor, a licensed master addiction counselor, or a licensed addiction counselor registered as a clinical supervisor.

2. Registration. An individual requesting to be registered for private practice shall make a formal application to the board and pay the registration fee.
