All requests for declaratory rulings shall be written and mailed to the Board of Examiners for Speech and Language Pathologists and Audiologists, P.O. Box 16885, Greensboro, North Carolina, 27416-0885. The request must include the following information:

1. (1) name and address of the petitioner;
2. (2) statute or rule to which petition relates;
3. (3) statement of the manner in which petitioner is substantially affected by the rule or statute or its potential application to him;
4. (4) a statement of whether an oral hearing is desired, and if so, the reason therefor.
