1. (a) The information required for an applicant to obtain a Certificate of Registration for a professional corporation or professional limited liability company organized to render professional counseling services shall include the following:

   1. (1) typed, or legibly printed, notarized application form;
   2. (2) proof of licensure as a licensed clinical mental health counselor or licensed clinical mental health counselor supervisor;
   3. (3) registration fee; and
   4. (4) a copy of the Articles of Incorporation or Articles of Organization.
2. (b) This Certificate of Registration shall remain effective until December 31 following the date of such registration.
