All requests for a declaratory ruling shall contain the following information:

1. (1) the name, address, and telephone number of the person making the request;
2. (2) the statute or rule to which the request relates; and

3. (3) a statement describing the manner in which the person has been or may be aggrieved by the statute or rule.

   If a hearing is desired, the request shall so state and shall include the reason a hearing is desired.
