Fees for massage and bodywork therapy establishments shall be as follows:
- (1) Application for license $20.00
- (2) Initial license fee 150.00
- (3) License Renewal 100.00
- (4) Late renewal penalty 75.00
- (5) Duplicate license 25.00
- (6) Inspection of establishment 150.00
