A complaint regarding a violation of the Practice Act or the rules in this Chapter shall be submitted in writing and shall document:

1. (1) the name of the licensed massage and bodywork therapist, licensed establishment, school, person, or other entity involved;
2. (2) a description of the alleged behavior or incident; and
3. (3) the name, mailing address, and phone number of the person filing the complaint.
