1. (a) In order to be acknowledged as having successfully completed a course of study as required by G.S. 90-629(4), an applicant for licensure must submit a transcript to the Board's administrative office.

2. (b) Such transcript must document that the applicant has completed all requirements in a course of study that meets the standards set forth in this Section, and shall indicate the following:

   1. (1) Passing grades in all courses;
   2. (2) Dates of attendance;
   3. (3) Date of graduation or successful completion of the entire program;
   4. (4) Total number of supervised classroom hours of instruction; and
   5. (5) Credential awarded, whether a certificate, diploma, or degree.
