Any listed qualified individual on any license may have his name removed from this license and obtain his own license upon meeting any current licensing requirements he has not previously met and submitting to the Board:

1. (1) his written request to remove his name from the license on which he is currently listed;
2. (2) the license certificate on which he is currently listed;
3. (3) his license application on a form furnished by the Board; and
4. (4) payment of the appropriate license fee.
