1. (a) The Form BAR-6 shall be filed by one applying to take an examination as a barber school instructor. It requires the following:

   1. (1) the name, address, social security number, and birthdate of the applicant;
   2. (2) the current registered certificate number;
   3. (3) the name of barber school attended; and
   4. (4) the proposed place of employment, if any.
2. (b) The fee in Rule .0101(a)(11) of this Section shall accompany this form.
3. (c) The Form BAR-6 shall include the applicant's attestation that the information in the form is correct.
