1. (a) Filers shall use forms promulgated by the Department.

2. (b) For each filing, a filer shall submit an original consisting of:

   1. (1) A completed and signed form;
   2. (2) A map which complies with Section .0400 of this Chapter, if the filing of a map is required;
   3. (3) Any attachments.
3. (c) For each filing, a filer shall also submit three copies of the documents submitted to Paragraph (b) of this Rule.
