A technology provider applicant shall disclose on its application if it or any of its key individuals is now or has ever been the subject of a debarment by a state, federally recognized tribe, or nation, and for each:

1. (1) whether the debarment was for the applicant or a key individual, and the name of the key individual;
2. (2) the name of each government that debarred the applicant or the key individual;
3. (3) an explanation of the reason for each debarment; and
4. (4) the start and end dates of each debarment.
