Notification to the Department pursuant to Rule .0208 of this Section shall include the following information when available:

1. (1) the names of the affected notaries public including their notary commission numbers;
2. (2) a description of the affected records, data, or solution;
3. (3) a copy of any law enforcement report made in connection with the incident;
4. (4) a description of how the reportable incident occurred;
5. (5) the duration of the reportable incident;
6. (6) a description of actions taken to prevent or mitigate a similar reportable incident;
7. (7) when the reportable incident occurred; and

8. (8) a point of contact for the technology provider who has knowledge of:

   1. (a) the reportable incident;
   2. (b) actions taken to address the reportable incident; and
   3. (c) actions to be taken to address the reportable incident.
