After authorization by the Department, a technology provider shall provide written notice to the Department:

1. (1) within five business days of changes to:

   1. (a) the information required by 18 NCAC 07B .0402(2);
   2. (b) the information required by 18 NCAC 07B .0402(3);
   3. (c) conversion to an alternate type of business entity;
   4. (d) conversion to a foreign entity;
   5. (e) failure to remain active and current with the Department's Business Registration Division;
   6. (f) merger with another business entity;
   7. (g) filing of a bankruptcy petition; or
   8. (h) a change in the technology provider's controlling ownership; and
2. (2) within 45 calendar days of changes to any other information on the provider's application for authorization.
