A tangible journal shall:

1. (1) be made in a bound journal into which pages cannot be inserted; and

2. (2) have on the first page:

   1. (a) the name of the notary public as commissioned;
   2. (b) the commission date of the notary; and
   3. (c) the county of the commission; and
3. (3) have sequentially numbered pages.
