Self-attestation by a principal of current location on a United States military installation or vessel outside the United States shall consist of:

1. (1) the name of the principal making the attestation to the notary public;

2. (2) whether the principal is:

   1. (a) a member of the Armed Forces of the United States; or
   2. (b) a dependent of a member of the Armed Forces, and the name of the member;
3. (3) the branch of the Armed Forces in which the member serves;

4. (4) that the principal is physically located outside the United States on:

   1. (a) a U.S. military installation; or
   2. (b) a U.S. military vessel; and

5. (5) unless Rule .0207 of this Section applies, the name of:

   1. (a) the installation and the country in which the installation is located; or
   2. (b) the name of the vessel; and
6. (6) that the principal is on the named installation or vessel pursuant to military orders assigning the member to active duty for more than 120 days.
