A certified notary instructor shall:

1. (1) comply with the institution's reporting requirements regarding suspected cheating; and

2. (2) report to the Department within two business days if the instructor has reason to believe that there has been cheating by any student on a notary course exam, and include:

   1. (a) the name and, if available, address and contact information, for any student suspected of cheating;
   2. (b) a narrative description of the instructor's reasons for believing cheating may have occurred;
   3. (c) whether, when, and to whom the instructor reported the possible cheating at the institution; and

   4. (d) any other information such as additional supporting evidence that the instructor believes may aid the Department in investigating the report.

      Note: For purposes of this Rule, "additional supporting evidence" includes evidence such as videos, student admissions, and witness reports.
