A notary public may release confidential information learned from the principals or records generated during a notarial transaction only if:

1. (1) all principals who participated in the notarial transaction agree;

2. (2) the request is provided to the notary in writing with a statement signed and dated by each principal with:

   1. (a) the printed name of each principal who participated in the transaction;
   2. (b) the date of the transaction;
   3. (c) a description of the types of notarial acts performed by the notary during the transaction;
   4. (d) a description of the confidential information sought; and
   5. (e) an instruction stating to whom the confidential information may be delivered; and
3. (3) the principal provides any other information that the notary may request for purposes of determining the information to be provided or to whom it is to be provided.
