A commission certificate:

1. (1) shall not be valid until the oath of office has been taken; and

2. (2) shall include:

   1. (a) the appointee's name for use on a notary public commission;
   2. (b) the county of commissioning;
   3. (c) the beginning date of the commission; and
   4. (d) the expiration date of the commission.
