If an individual was denied a public office, the individual shall include with the application:

1. (1) the name of the public office that was denied;
2. (2) the locality, state, federally recognized tribe, or nation where the public office is or was located;
3. (3) the date of the denial;
4. (4) the name used when the denial occurred;
5. (5) the reason for the denial; and
6. (6) other information that the individual wishes to include in order to aid the Department in assessing the individual's qualification for authorization.
