An individual required to provide a criminal history record pursuant to Chapter 10B of the General Statutes and the rules in this Chapter shall include:

1. (1) a complete listing of felony convictions of the individual and the name under which each conviction was entered;
2. (2) a complete listing of misdemeanor convictions of the individual and the name under which each conviction was entered;
3. (3) if the individual's criminal history record includes a conviction in North Carolina, a copy of the individual's criminal history record prepared by the North Carolina State Bureau of Investigation;
4. (4) if the individual has a criminal conviction in a jurisdiction other than North Carolina, a copy of the individual's criminal history record from a criminal history record registry or repository of record reflecting the conviction; and

5. (5) the signed explanation required by Rule .0503 of this Section.

   Note: A "registry or repository of record" is the official governmental state, federal, national, or tribal archive or center for collection of criminal history information.
