The form used by a manufacturer or vendor of physical notary seals to register with the Department requires:

1. (1) the information required by Rule .0402 of this Section, except that the residential address shall not be required for a key individual operating a sole proprietorship or a general partnership;

2. (2) certification that the manufacturer or vendor:

   1. (a) manufactures or sells physical notary seals to North Carolina notaries public; and

   2. (b) complies with, and will continue to comply with, the requirements of:

      1. (i) G.S. 10B-36 and 10B-37; and
      2. (ii) the applicable rules in 18 NCAC 07G; and
   3. (c) the manufacturer or vendor agrees that it will comply with the rules in 18 NCAC 07G;
   4. (d) the information provided on the form is true and complete;

3. (3) signature by an individual with the authority to bind the manufacturer or vendor with:

   1. (a) printed name and title of the individual; and
   2. (b) the date signed.
