The form used to apply for authorization as a licensed platform requires:

1. (1) the information required by Rule .0422 of this Section;

2. (2) for each of the technology provider's key individuals:

   1. (a) the information required by 18 NCAC 07J .1505;
   2. (b) a brief description of the individuals' duties; and
   3. (c) the information required by 18 NCAC 07B Section .0700; and
3. (3) the information required by 18 NCAC 07J .1501.
