The notary instructor application for recertification form requires:

1. (1) the information specified in Rule .0414 of this Section;
2. (2) verification that the applicant has taught the notary instructor course at least twice a year during the current two-year certification period;
3. (3) the date of initial certification as an instructor;
4. (4) whether certification has been continuous;

5. (5) a declaration under penalty of perjury that:

   1. (a) the information provided is true and complete;
   2. (b) the applicant remains qualified to be commissioned as a notary public; and

   3. (c) the applicant:

      1. (i) understands the official duties and responsibilities of a notary public and notary public instructor in North Carolina; and
      2. (ii) will perform to the best of the applicant's ability all responsibilities of teaching the notary courses in accordance with the law.
