The student evaluation of notary instructor form requests the following information from a student:

1. (1) the name of the instructor;
2. (2) the date of the course;
3. (3) the name of the institution;

4. (4) rating of the instructor's:

   1. (a) professionalism;
   2. (b) subject matter knowledge;
   3. (c) use of audio-visuals, handouts, and other materials; and
   4. (d) responsiveness to questions from students;
5. (5) rating of the course materials;
6. (6) other information or comments that the student wishes to add; and
7. (7) the student's name and contact information if the student is willing to be contacted by the Department with follow-up questions.
