The recommendation of notary instructor applicant form requires:

1. (1) the instructor applicant's commission name;
2. (2) the name and contact information for the individual completing the form;

3. (3) the individual's basis for recommending the applicant, including:

   1. (a) a declaration that the individual is not a family member of the applicant;
   2. (b) how the individual knows the applicant; and
   3. (c) whether and how the individual has knowledge of the applicant's teaching skills;
4. (4) whether, in the opinion of the individual, the applicant has the skills to be an effective teacher of notary applicants;
5. (5) a declaration that the individual personally recommends the applicant to be a notary instructor;
6. (6) the printed name and signature of the individual and the date signed; and
7. (7) a declaration under penalty of perjury that the information provided is true and complete to the best of the individual's knowledge and belief.
