The certificate of appointment form includes:

1. (1) space for the entry by the Register of Deeds in the county of the commission to enter:

   1. (a) the name of the individual administering the oath;
   2. (b) the title of the individual administering the oath;
   3. (c) the name of the appointee who appeared before the individual and took the oath; and
   4. (d) the signature of the individual who administers the oath; and
2. (2) space for the signature of the notary public after the oath has been administered.
