All applicants for a North Carolina educator license shall provide the following basic entity data through the North Carolina Department of Public Instruction's secure, online licensure system:

1. (1) Social Security Number;
2. (2) Full legal name;
3. (3) Date of birth;
4. (4) Mailing address;
5. (5) Telephone number; and
6. (6) Non-school email address.
