The following fees are required for well contractor certification applications, renewals and temporary certifications:

1. (1) Annual Fee: This fee shall accompany each new application for certification or renewal of certification. The annual fees are as follows:

   Level A: two hundred dollars ($200.00)

   Level B: one hundred fifty dollars ($150.00)

   Level C: one hundred twenty five dollars ($125.00)

   Level D: one hundred dollars ($100.00)

2. (2) Examination Fee: A fee of fifty dollars ($50.00) shall accompany each request for examination. Where an applicant requests an examination to be administered at a time other than a regularly scheduled examination, the fee shall be one hundred dollars ($100.00).
3. (3) Temporary Certification: A fee of one hundred dollars ($100.00) shall accompany each application for temporary certification.
