All material changes to an approved contract form shall be filed with the Division for approval before use. For the purpose of this Rule, a "material change" includes a change in:

1. (1) the means of calculating payment to the provider; for example, change from fee for service to capitation;
2. (2) the distribution of risk between parties; or
3. (3) the delegation of clinical and administrative responsibilities.
