The renewal license fee set forth in G.S. 58-6-7 shall be paid annually to the commissioner before March 1 of each year. The renewal license application shall include, but not be limited to:

1. (1) the name and address of the organization,
2. (2) the lines of insurance in which the organization engages,
3. (3) a description of pertinent changes in its services of activities since the previous renewal license was granted.
