The certification form required by G.S. 58-84-46 shall be entitled "Local Relief Fund Board of Trustees Report" and shall, include the following:

1. (1) the name of the city, fire district, or sanitary district;
2. (2) names, home addresses, email addresses, and phone numbers of the "board of trustees of the local Firefighters' Relief Fund";
3. (3) identify of the treasurer and board chair of the local Firefighters' Relief Fund; and
4. (4) the date each person was (re)elected or (re)appointed to the board.
