When an insurer makes a written response to an inquiry by the Division or complaint made by a consumer, the insurer shall identify on its response:

1. (1) its mailing address;
2. (2) official corporate name of the company against which the complaint is made;
3. (3) the NAIC company code; and
4. (4) specific corporate name if the insurer is part of a group of companies.
