1. (a) The agency shall document information pertinent to meeting conditions of eligibility in the individual's service record.
2. (b) The agency shall document the eligibility decision, the date of the decision and the service worker/case manager making the decision on the application form.
3. (c) The agency shall document the beginning and ending date for all services provided with regard to income.
4. (d) For all services provided without regard to income, except for child care services, the agency shall document the beginning date, which is the eligibility date.
